Brief Description
The Overall Anxiety Severity and Impairment Scale (OASIS) is a brief measure of severity and impairment related to anxiety. The measure can be used across anxiety disorders, with co-occurring anxiety disorders, and with subthreshold symptoms of anxiety. The OASIS is particularly useful for differentiating between those with significant anxiety symptoms from those without (see scoring information below). Respondents rate the severity and impairment of anxiety symptoms during the past week on a scale of 0 to 4, with each response option providing detailed descriptions of severity and impairment associated with symptoms.

Summary of Scoring and Interpretations
The OASIS contains 5 items that are scored on a 5-point Likert scale from 0 (lowest severity and impairment) to 4 (highest severity and impairment). A total score is calculated by summing the item responses, with higher scores being associated with more severe and more impairing anxiety.
The Blueprint OASIS measure uses the cut-off score of 8, meaning that a total score of 8 or more suggests significant symptoms of anxiety. Bragdon and colleagues (2016) recommend the following cut off scores:
