Brief Description
The Obsessive Compulsive Inventory - 12 (OCI-12) measures OCD symptoms, assessing OCD’s four main symptom dimensions of checking, ordering, washing, and obsessing. It is derived from the 18-item OCI-Revised (OCI-R), which had hoarding and neutralizing as additional symptom dimensions. The OCI-12 can be used as a screener or as a way to monitor treatment progress. Respondents rate how often they have been distressed or bothered by the OCD symptoms listed (e.g., “I check things more often than necessary”) on a scale from “Not at all” to “Extremely.”

Summary of Scoring and Interpretations
The OCI-12 contains 12 questions scored on a 5-point Likert scale with values from 0 (“Not at all”) to 4 (“Extremely”). A total score is calculated by summing the item responses and ranges from 0 to 48; higher scores are associated with higher levels of, or more severe, OCD symptoms. Scores of 11 and above are clinically significant and indicate the likely presence of OCD. See below for score interpretations.

Hoarding was once considered to be a main symptom dimension of OCD (and was included as such in the OCI-R); it is now classified as its own disorder.
There are other versions of OCI, including the original 42-item version, revised 18-item version, and short 4-item version.
