Brief Description
The DSM-5 Level 2—Sleep Disturbance—Child Age 11–17 is an 8-item assessment that evaluates sleep disturbances in children and adolescents over the past week. This measure is based on the PROMIS Sleep Disturbance Short Form and is designed to assess the severity of sleep problems, such as difficulty falling asleep or staying asleep, and overall sleep quality. In the first seven items, respondents are asked to identify the frequency with which they experienced sleep disturbance in the past week (e.g., “In the past seven days, I had difficulty falling asleep”). Responses are provided on a 5-point scale, ranging from “not at all” to “very much.” The final item asks respondents about their perception of their overall sleep quality in the past week, with responses ranging from “very poor” to “very good.” The DSM-5 II Sle-C is typically completed by the child before meeting with a clinician and is best used as a tool to monitor sleep issues that may affect mental and physical health. The measure can be repeated over time to track changes in sleep disturbance severity, providing insight into the effectiveness of interventions or the progression of symptoms.

Summary of Scoring and Interpretations
Responses are provided on a 5-point Likert scale from 1-5. Items 2, 3, 7, and 8 are reverse-scored. Scoring the measure involves summing the raw scores from each of the 8 items, with total scores ranging from 8 to 40. Higher scores indicate more severe sleep disturbances. Consistently high scores may signal areas requiring further assessment or follow-up care, with clinical judgment playing a key role in interpreting the results.
