Brief Description

The DSM-5 Level 2 Substance Use-Adult (DSM-5 II Sub Use) measure is an assessment tool adapted from the NIDA-Modified ASSIST and is often used as a follow-up to the DSM-5-TR Level 1 Cross-Cutting Questionnaire. The DSM-5 II Sub Use is a 10-item measure that assesses the frequency of prescription medicine and illicit substance use over the past two weeks. The measure is typically completed by the individual, but it can be administered by a knowledgeable informant if the individual has impaired capacity. Each item asks the respondent to rate the frequency of use for various substances, such as painkillers and heroin, with possible responses ranging from "Not at all" to "Nearly every day."

Summary of Scoring and Interpretations

Blueprint instructions differ from the original version in that they do not acknowledge that this measure is being assigned as a follow-up due to a positive result from the DSM-5-TR Level 1 Cross-Cutting Questionnaire. This allows for clinician flexibility to assign the measure as appropriate in a variety of clinical contexts.

Blueprint provides a total score. However, this score has little to no relevance as each item should be looked at separately. Thus, each item is represented as its own individual subscale with scores of 1 or greater recommending further follow-up/assessment.
