Brief Description
The Satisfaction with Care (CARE) assessment is a 1 question survey that assesses a client’s overall satisfaction with the care they received. The question asks, “How satisfied are you with your care experience?” on a scale from "Very Unsatisfied" to "Very Satisfied."

Summary of Scoring and Interpretations
The CARE assessment contains 1 question scored on a 5-point Likert scale with values from 1 (“Very Unsatisfied”) to 5 (“Very Satisfied”).
Blueprint Notes
This survey will automatically be administered when you are discharging a client from your services in Blueprint and is not found under the Assessments list. When discharging a client, clients are automatically sent the end of care assessments, which includes the CARE assessment along with their assigned assessments one last time. The CARE survey will expire one week after it’s been sent. You can opt to not send the end of care assessments by deselecting the “Send end of care assessment to client” box. See screenshot below:
You can view the results in the client’s profile alongside their other assessment results. Additionally, the responses will be added to the Client Satisfaction Report, which can be found under the Reports tab in the blue banner up top.

If you want to send only the CARE survey and not the client's assigned assessments one more time, go to the client’s profile and un-assign any assigned assessments under the Assessments tab. Then discharge the client and ensure the “Send end of care assessments to client” is selected.
