Instructions for completing Check-ins and Assessments using the Blueprint Client Mobile App

Check-ins and Assessments are two ways to gather and deliver information related to your treatment from you to your Clinician. This information helps your Clinician measure outcomes and results over time so they can form make informed treatment decisions. πŸ€—

Below we will walk through how to complete a Check-in and Assessment using the Blueprint Client Mobile App.

How to Complete a Check-in using the Blueprint App

  1. Open and login to the Blueprint app

  2. Click Check-in in the upper-right

  3. Click Start next to the Check-in you'd like to complete OR click Start all modules at the bottom of the page to complete all Check-ins assigned to you
    ​

  4. Go through all questions clicking Next after answering each one.
    ​

  5. Click Done on the final page

  6. That's it! πŸŽ‰ Your Check-in results will be sent to your Clinician and added to your Dashboard.

    πŸ’‘ Some Check-ins (Lifestyle, Location & Mobility, Physical Activity) must be completed multiple times or multiple days of GPS access before the graph populates. If this is the case, you'll notice a message like the one in the image below:

How to Complete an Assessment Using the Blueprint App

  1. Open and login to the Blueprint pp

  2. If you have an Assessment due, you'll see a red banner at the top, prompting you to take the Assessment(s).
    ​
    ❗️If you do not see this banner, that means you do not have an Assessment due at this time and will not be able to complete one until it is due.
    ​

  3. You'll now be guided through the Assessment(s) you have due, question-by-question.
    ​

  4. That's it! πŸŽ‰ Your Assessment results will be sent to your Clinician upon completion.

  1. Q: What's the difference between an Assessment and a Check-in?
    ​
    ​A:

    • Check-Ins:

      1. A way for you to touch base with your Clinician by recording thoughts and/or feelings outside of official Assessments using Symptom Trackers, Worksheets or Journals

      2. Completed done on a frequent (usually daily) basis

      3. Not billable to insurance or Client

    • Assessments:

      1. Official, validated symptom rating scales (such as the PHQ-9, GAD-7)

      2. Frequency is determined by Clinician -- usually completed on a weekly basis

      3. Billable to insurance and may sometimes have a Client out-of-pocket cost associated with them, depending on your insurance plan or your Clinic's policy.

  2. Q: How often should I complete a Check-in?
    ​
    ​A: Check-ins were designed to be completed daily, but talk with your Clinician to determine your Check-in frequency based on your individual treatment plan. πŸ€—
